What are the most common database design mistakes?

In a 2008 eWeek slide show, Brian Prince identified "The Most Common Database Design Mistakes." The list was based on input from analysts and some professionals in the field. Prince's list includes 1) lack of planning, 2)failure to adopt a naming standard for tables and fields, 3) too many or too few indexes, 4) lack of stakeholder involvement, 5) not using tools for database tuning, database optimization or to assist in maintaining data integrity, 6) poor design and lack of normalization for transaction systems, 7) poor data back up or poor RAID implementation, and 8) ignoring the physical data model.
